- Your name, date of birth and address
- Driver's license number, or the state that suspended it
- The vehicle — or tell us you do not own one
- The reason for the filing: DUI, no insurance, points or other
What happens after you accept
The policy is issued, the SR-22 is transmitted electronically to the state, and you get the confirmation by email. There is no trip to a DMV office to hand in paper.
